Why and When

  • Does your elderly relative need rehabilitation following a long hospital admission?

    Do you feel you or a family member would benefit from exercises to help with walking and completing day to day activities?

     

    Are you waiting a number of weeks for NHS community services to see you at home?

    Home visits can be arranged (including sheltered accommodation and residential/nursing homes) with a fully qualified and specialised physiotherapist.

During Your Visit

During our visits you can expect the following:

  • Full physiotherapy assessment looking at how well you move, how strong you are, how well you walk, how good your balance is and how you get to and from places (out of chairs, out of bed etc).
  • Full falls risk assessment (including environmental assessment) based on NICE guidelines 2017.
  • Mobility Aid Assessment.
  • Individualised specific exercise programme’s which aim to improve any deficits found during assessments.
  • Gait re-education.
  • Balance exercises.
  • Advice on how to reduce falls, pacing, energy conservation and specific breathing techniques if you get short of breath.
  • Signposting to local services to help provide a holistic approach to therapy
